Animal Surrender Form

There are many factors that go into our ability to intake an owner-surrendered animal. Please complete the form below for consideration.

Animal Surrender/Intake Fee: $150

If ASAP Rescue is able and agrees to intake your surrendered animal, you will be required to show proof of legal/recorded ownership of that animal. This can be through a microchip, veterinary records, paperwork, etc.
